Intensive Case Coordinator

Reporting directly to the Sohkayawin Housing Manager, the Intensive Case Coordinator delivers housing stabilization and intensive case management services to tenants within the apartment complex. The role involves working in a high‑risk environment with individuals experiencing complex challenges, including mental health concerns, substance use, and histories of trauma. The Coordinator provides a holistic, trauma‑informed, and culturally responsive approach to care, coordinating team efforts and ensuring tenants maintain stable housing and improved overall wellbeing.
