Position Overview We are hiring an experienced Home Health RN to provide skilled nursing care in a field-based setting . This role focuses on chronic disease management, wound care, and reducing hospital readmissions through patient-centered care.
Key Highlights - Setting: Home Health (field-based)
- Schedule: Monday-Friday, 8:00 AM - 5:00 PM
- Patient Load:
- ~6-7 units/day (~30/week)
- Admissions weighted at 2.4 units (~2 admissions/day)
- Traveler Friendly: Yes (must have Home Health RN experience)
Patient Population - Adults (18+)
- Common conditions include:
- Wound care & wound vacs
- Diabetes, CHF
- Post-acute care
