Department: Patient Flow

The Registered Nurse in Patient Flow will support the philosophy that each patient receives the right level of care, in the right place, at the right time by the right provider. Responsibilities of this position include but are not limited to:

• Addressing identified processes and/or other issues impeding the patient’s flow from admission to discharge.
• Coordinating the multidisciplinary team‘s plan of care to make sure needs are met promptly, and ensuring consistent messaging to the patient and family regarding discharge plans is ensured
• System navigation and bed placement
• Building awareness of the patient navigator role/services among the health care team is important to assist in coordinating patient care and identifying “at‑risk” patients that need navigation services.
