Conviva Senior Primary Care, a subsidiary of Humana Inc., is recruiting a full-time primary care physician for our Orange Park center in Jacksonville, Florida. We have a unique, value-based care model and we are experts in senior-focused primary care. As a Primary Care Physician, you will report to our Chief Medical Officer/Regional Medical Director.

**A day in the life of a Conviva Primary Care Physician:**

+ Monday-Friday 8:00am-5:00pm
+ Join an experienced team with an established patient panel
+ Opportunity to spend more time with patients
+ Spend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time as it relates to direct patient care
+ Minimal, shared call rotation

+ Evaluates and treats center patients in accordance with standards of care.
