Job Description

The Nurse Practitioner (NP) is a nurse who has an advanced level of clinical nursing practice that maximizes the use of graduate educational preparation, in-depth nursing knowledge and expertise in meeting the health needs of individuals, families, groups, committees and populations. The NP brings a holistic approach to care, addressing mental, physical and social aspects. Practices independently within the legislated scope of practice.

Responsibilities

• Monitors ongoing conditions for patients with stable and chronic illnesses.
• Provides family support, education and counseling on diagnosis, treatment options, evaluation outcomes and follow-up plans with a focus on health promotion, illness prevention and timely reintegration into the community.
• Works with community partners in planning, treatment, discharge planning and community reintegration.
