Job Description

• To escort patients to and from appointments in a safe and timely manner

• Accompany patient on trips to/from scheduled appointments and remain with the patient at all times

• Ensure any documentation/appointment information etc. is provided to the facility's nursing supervisors

• Inform facility of any changes in residents situation / plan

• Must be able to communicate well with the patient to assure a positive experience.

Minimum Qualifications:

CNA / HHA Certification
