Position Details
Duties
: Support triage, assessment, and follow-up care for individuals presenting in crisis.Works in partnership with the program psychiatrist to deliver short-term treatment plans for clients who meet appropriate criteria, working independently with appropriate oversight from the psychiatrist. Integrates clinical findings and evidence-based practice to formulate diagnoses and develop initial care plans.Collaborates with patients and members of the interdisciplinary team to identify and implement pharmacological and non-pharmacological interventions that promote optimal outcomes.Participate in the dissemination and utilization of best practice guidelines.Provide education, mentorship and support to the health care team, primary care providers, hospital partners, and community groups.Advocates for supportive and safe client care as well as collaborative practice and utilization of evidence-informed pr...
