Resident Care & Support

In this role, you’ll be at the forefront of clinical care:

• Collaboratively provide care with our interdisciplinary team members, including leading and supporting Registered colleagues in all clinical matters.
• Use your expertise to conduct thorough health assessments and collaborate with physicians on diagnosing acute and chronic conditions.
• Work closely with residents, families, and friends to conduct resident-centered reviews, evaluations and adjustments to care and their individual plans.
• Regularly engage residents in dialogue to determine what is important to them for health and quality of life.
• Order and interpret diagnostic tests, and use this information to make informed clinical decisions.
• Provide data and insight for High Intensity Needs Submissions, shaping care initiatives.
• Collaborate with external parties such as hospitals including Acute Care, LTC Hub, out reach teams, ...
