The Broadway Practice has transformed how we provide care to the Washington Heights community through its successful pilot of the Interdisciplinary Care Team, core to the Patient-Centered Medical Home model. All members of the care team-Nurses, Nutritionist, Care Manager, Community Health Worker, Patient Financial Assistants, Social Worker, Medical Assistants, and Physicians collaborate to improve the health and well-being of our patients.
