The Abbotsford Regional Hospital Heart Function Clinic (ARH HFC) is an outpatient clinic where a multidisciplinary team manages patients with a diagnosis of heart failure or suspected heart failure with or without low ejection fraction using evidence‑based care. These patients have complex medical needs and will require patient education and optimization of therapy through medications over multiple visits. Once optimized, patients are returned to their primary care providers or specialists for ongoing care.
