The Nurse Practitioner (NP) provides direct, specialized, comprehensive nursing assessments, case management, lifestyle counseling, and education emphasizing health promotion and disease prevention strategies to positively affect health outcomes. The NP works in an extended nursing role demonstrating a high level of autonomy and expert skill in order to formulate clinical decisions, appropriately manage acute illness and promote wellness within their area of specialty.
The NP explicitly consults the inter‑professional team where care is required outside of the NP scope of practice and/or not covered by a Medical Directive. The incumbent acts as a liaison and consultant to other health care providers and regional partners regarding patient care and health services within the LHIN. She/he provides leadership in the transfer of knowledge into practice in nursing and related fields appropriate to the clinical focus.
