Make a meaningful impact every day as a CenterWell Home Health Nurse. You will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients get back to the life they love. You will report to the Clinical Manager.

As a Home Health LPN, you will:

+ Provide skilled nursing interventions in the treatment of the patient's illness, rehabilitative needs and preventative care, under the direction of a Registered Nurse
+ Confer with the RN Supervisor regarding needed changes in the Plan of Treatment
+ Maintain contact with patients, physicians, clinical manager(s), other members of the healthcare team regarding patient/family needs and status changes
+ Participate in care coordination activities and discharge planning
