What you’ll do:
We are seeking a dedicated Pharmacist to join our team in a full-time, permanent capacity. This position plays a key role in providing high-quality clinical pharmacy services to Long Term Care (LTC) facilities. The successful candidate will work closely with healthcare teams to ensure optimal medication management and patient outcomes.
Provide clinical oversight for LTC pharmacy services, including medication reviews and care planning. Monitor drug therapies for efficacy and safety; recommend adjustments as needed.Collaborate with nursing staff, physicians, and other healthcare professionals in LTC facilities to ensure appropriate pharmaceutical care as a representative of Discovery Co-operative Ltd.Provide safe, courteous, efficient and customer-focused service in our Pharmacy Department.
