As a Family Support Counsellor I within the Acute @ Home program, you will provide essential family support services to children and their families. Your role involves in-home support, school liaison, and community coordination as needed. With a keen focus on quality service delivery, you will also ensure compliance with Wood's Homes' Health and Safety protocols.
Key Responsibilities: • Conduct assessments and establish personalized goals • Advocate for families and develop community resources • Provide parent skills training and family counselling • Intervene during crises and follow up on care • Participate in clinical rounds and team meetings
