**Overview**

The Clinical Review Pharmacist shall assist the Supervising Pharmacist, Prior Authorizations in formulary management including, but not limited to: overseeing the prior authorization process, utilization review reporting, pharmacy and therapeutics committee activities, communication with physicians and pharmacists, administration of appeals and grievances, and communication with staff on departmental policies and procedures. On call duties are a requirement of the job.

**Qualifications**

+ Bachelor’s degree required; licensed to practice pharmacy in the state of New York required.
+ Five (5) years of pharmacy experience required; professional experience providing pharmaceutical care, preferred.
