**EDUCATION & EXPERIENCE**

**Minimum Qualifications:**

+ Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
+ Completion of an ASHP accredited post-graduate year 1 residency program or 3 years of related experience.

**LICENSES, REGISTRATIONS OR CERTIFICATIONS**

**Required:**

+ Licensed to practice pharmacy in the state of Texas or eligible for licensure from the Texas State Board of Pharmacy.
+ Must be licensed with the Texas State Board of Pharmacy within 90 days of hire.

**Preferred Qualifications:**

+ Doctor of Pharmacy degree
+ Completion of an ASHP accredited post-graduate year 2 residency program in Internal Medicine
+ Experience as a Pharmacist in Internal Medicine
