The Mississauga Halton Diabetes Foot Care program is a regional program providing preventative foot care to high risk patients with diabetes who are facing barriers to accessing fee for service care models. The Chiropodist serves as both a mentor and clinician in this regional program providing foot care to high risk patients with diabetes. This position is part of a regional approach to addressing the needs of this population and has the goal of demonstrating a decrease in admissions for diabetes related wound care. The team will include nurses and chiropodists. The Chiropodist will provide foot care services in the region at various community settings within a geographic area.
