Job description

We are looking for an Adult's Social Worker to join a Mental Health Hospital Discharge Team.

This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.

About the team

This is a fast paced team who work to ensure mental health patients are safely discharged from hospital and have as much support as necessary. Carrying out bedside assessments, arranging appropriate care packages for comprehensive discharges and advocating for service users and their independence is at the forefront of the team's motivation. Being able to ensure service users have access to necessary resources and external support at the earliest stages is key to the success of this role.

About you
